The fork truck was overloaded by 25% to 50%. Based on the amount of people around it this is SOP.

Once the lift reached 3 to 4 feet a tipping point was achieved.
As the forks tipped the load shifted forward and dropped.
Resulting in the truck returning to its normal upright position.

The person who jumped on the back, jumped on just as the unit was about drop down.
Lost the grip and slid under.

The corrective action for the load would have been to lower the forks as soon as it began to tip.

When lifting a heavy load on a fork truck you lift slowly up to and past the tipping point so you can drop the lift if you tip.

I will be honest in some environments driving a fork truck can be a real pain in the ass.
